New patient forms

If you are a new patient requesting treatment for addiction please click on and complete all 4 parts below.  This is your new patient paperwork including your reason for visit, medical history, as well as our policies and procedures.  If you are seeing us for Psychiatry only and not addiction you can omit the "Opioid Use History" form.  You must complete all 4 parts to be seen otherwise.
